The following Wellness Program Incentives are available to TAC GHB Wellness Program participants. The best incentive for participating in the Wellness Program is achieving your optimal health and wellbeing! But as we know, change can be a challenge. The Texas Annual Conference of the United Methodist Church (TAC) offers cash incentives to encourage and reward participants for achieving their specific wellness goals.

Weight Loss Incentive
The Weight Loss Incentive is available to you once you have achieved your target weight loss goals. You can apply for a single incentive or multiple incentives at a time; however, each incentive can only be earned once. All weight calculations are based on your starting weight recorded at the Day of Wellness. All wellness incentives are based on a target Body Mass Index (BMI) of 25.0. For more information, download a BMI chart. Please note that the total of all Wellness Incentives earned in any one calendar year, January 1 thru December 31, are limited to $2,000 per person.
You can earn up to $1,000 by achieving the following weight loss goals:

$200 - Losing 5% of your total weight recorded at the Day of Wellness.
Incentives 2 and 3 below require confirmation of current weight and height and date of your annual physical examination. Please use the attached Physician Confirmation Form.

$200 – Losing 10% of your total weight recorded at the Day of Wellness.

$600 – Losing 100% of the weight required to achieve your target body mass index (BMI) of 25.0.
Complete and submit the Weight Loss Incentive Form & Physician Confirmation Form in a PDF format preferably by email.
Please allow 4 to 6 weeks for processing and mailing of your incentive check. You will receive a 1099-MISC for all Wellness Incentives paid in a calendar year.
2024 Annual Maintenance Incentive
An Annual Maintenance Incentive of $1,000 can be earned by maintaining a BMI of 25.0 or lower as specified below:

If a participant is at a BMI of 25.0 or lower at the Day of Wellness and maintains a BMI of 25.0 or lower, the 2024 Annual Maintenance Incentive can be applied for starting 6 months from attending the Day of Wellness and no later than December 31, 2024.

If a prior incentive has been earned, the 2024 Annual Maintenance Incentive can be applied for starting 6 months from the date of the most recent incentive form submission and no later than December 31, 2024.

Only one $1,000 Maintenance Incentive will be paid during 2024. Please note that the total of all Wellness Incentives earned in any one calendar year, January 1 thru December 31, are limited to $2,000 per person.

Pregnancy Weight Loss Incentive
Women enrolled in the TAC GHB Wellness Program are eligible to earn the Pregnancy Weight Loss Incentive. The mother must be a participant in the Texas Annual Conference Group Health Benefits Program when the baby is born. Women who return to their 1st trimester weight in the year following their delivery will be eligible for a $1,000 incentive. This incentive is in addition to other earned incentives. Please note that the total of all Wellness Incentives earned in any one calendar year, January 1 thru December 31, are limited to $2,000 per person.
