1. Describe the collaborative listening/learning aspects of your engagement with this community that led to this proposal, if any.

2. Describe your relationship with this community, including, but not limited to, how many years you have been working with this community, how you have helped to empower the community toward self-sustainability, how this community has served in leadership with you and how you have utilized the resources within the community in your work on behalf of the community and/or local church.

3. How many of the past 5 years has your church or organization received funding from the Texas Annual Conference through Partners in Mission or otherwise for mission projects and/or teams?

4. Please describe your planned activities while in the community, including how you will be working with local community members and elevating their leadership. If you will be doing work that local community members could be employed to do, please give your justification for doing that work yourselves. Could they be employed to do the work if money was raised for them to be paid? What skills are you teaching that the local community can subsequently use going forward? What will your team be learning from the local community and how will you use those skills in your local community, church or organization upon your return?

6. How many team members from your church or organization will be working on this project? How many of those team members are first time participants?

In order to foster discipleship among our youth and young adults, are you willing to consider giving a scholarship to a college student who could not afford an opportunity like this and who is not a member of your church or organization? Would your team be able to accommodate a college-aged team member who has a scholarship from another church or entity?

8. Are you willing to commit to a long-term relationship with this community and to explore ways that members of this community can engage in mission with you? Explain how you will do that.
